upload images: bot hin "insert image" and "manage attachments"

after I've selected a file to upload and clicked "upload", it looks like a black bar comes up & hides the selections.

nothing happens (no upload)

I think this black bar is hiding something because sometimes if I position the pointer just above the bar & a little to the left it looks like something is changing color under there

I have a problem uploading attachments too. When I click on upload photo, it only gives me the choice of adding a url. Can't share my own documents from my desktop. Apparently a different type of box is supposed to show up when you click that icon but I don't get it.
